DO NOT PROCEED WITH THE 15-PASSENGER VAN SAFETY AWARENESS COURSE UNTIL:
1- The top portion of the 15-Passenger Van Driver Approval Form (including signature, date, personal information box) is completed.
and
2- You have completed a review of your driver's license with University Police as indicated by a signature or initials in the appropriate space on the form.
