Guide for Review of Reports
  1. Background
    1. Is the identifying information complete and accurate?
    2. Is the appropriate referral information included?
    3. Are the sources of information reported for all statements in the history?
    4. Is all of the information included relevant to the current report?
  2. Evaluation
    1. Are all aspects of observation addressed?
      1. oral structure and function
      2. hearing screening
      3. receptive and expressive language
      4. phonology/articulation
      5. fluency
      6. voice
    2. Are tests explained as necessary?
    3. Are raw scores explained as necessary?
    4. Are behaviors described?
    5. Are informal observations descriptive?
    6. Are interpretations avoided?
  3. Diagnosis
    1. Is there a severity rating?
    2. Is it clear that the TEST didn't make the diagnosis?
  4. Prognosis
    1. Are the rationale and conditions for the prognosis included?
  5. Recommendations
    1. Are the recommendations stated as suggestions and not orders?
    2. Are appropriate referrals made?
